To obtain a replacement certificate please complete the following form, which will open in MS Word format so it can be downloaded and completed.

Replacement Certificates are £30 plus VAT (total £36.00) so please include a cheque made payable to PAA\VQ-SET with your request. If you are from a PAA\VQ-SET Approved Centre and would prefer to be invoiced for this please contact us.

Please provide as much information as possible to help us trace your original certificate. Please also ensure that the form has been signed and Page 2 Confirmation of Identity and Claim has been completed . If we are unable to issue a replacement for any reason your cheque will be returned.
